Download the official Prize Claim Form and Declaration Form (valid from April 2024) and fill it out completely.
Submit the Necessary Documents
Ensure you have the following documents ready:
The original winning ticket along with a photocopy, signed by a Gazetted Officer or Notary Public.
An affidavit proving ownership of the winning ticket, issued by a 1st Class Magistrate or Notary Public.
Your PAN card, ID proof, 4 passport-size photos, and bank details (including a cancelled cheque).
