Fill Out the Claim Form: Download and fill out the prescribed Prize Claim Form and Declaration Form, effective from April 2024.
Submit Required Documents: Original winning ticket with a photocopy attested by a Gazetted Officer/Notary Public.
Affidavit of ownership of the prize-winning ticket from any 1st Class Magistrate/Notary Public.
PAN card, ID proof, four passport-sized photographs, and bank details (including a cancelled cheque leaf).
